Diagnostic Assessment
Diagnostic Assessments are available to children and families once it has been
determined through an initial screening that the youth has mental health or
addictive disease needs. The initial Diagnostic Assessment and resulting
Individualized Recovery Plan or Individualized Resiliency Plan are required
within the first 30 days of service along with the ongoing Diagnostic
Assessments. The Diagnostic Assessment and Individualized Recovery Plan or
Individual Resiliency Plan process consists of a face-to-face comprehensive
clinical assessment with the individual and must include the individual’s
perspective. The assessment should also include family members and significant
others, collateral agencies and treatment providers.
